Yeah, I would tell her. I would tell her quietly though, at a time it would be convenient for her to sneak off to the restroom and fix it. I would hope someone would tell me something like that in the same manner. I was really grateful once when a coworker told me blood had soaked through the back of my skirt, in time to get some cold water on it.
The kind of thing I would not point out is something she could not do anything about, like a run in her stocking or a stain on her clothing.

Yes, I would tell her. It's a courtesy and I don;t want her to feel embarrassed more than necessary. I;m a clumsy person myself, so I can imagine that if I were in her shoes, I would want someone to point it out for me, and it is better a woman than a man. After all, it only takes very little effort to let her know and you will help her a lot that day.

Yes, I would pull her aside and tell her. I've probably worked with her for awhile and if I did something like that, I sure would want someone to tell me. Now if it was a stranger at a party or something, I probably would just let it go or tell someone that knew her better. :-)

I'd tell her in a quiet and respectful way. It's much better than not letting her know. While she'd probably still be embarrassed, I still think it's better than her not realizing it at all and going on with her day. I like what charray7 said too; if I didn't know her well, telling one of her friends would be another great way to go.

If possible, I would probably prefer to quietly slip her a note very quickly or whisper to her from the moment I noticed it. I'd want to act before many other people could notice her shirt was inside out, but yet keep any attention off of her so she'd have a chance to go into the restroom and deal with it.
Also, I could make up some excuse for needing her help with something, and get her out of the area to let her know privately.
I don't usually pay much attention to such things as what others are wearing, so if I noticed it at all then that would mean there was still a chance that most other people hadn't yet realized that anything was different, and perhaps I could help the woman before they noticed.
