Silly answer:
I would save the world, it is where I keep all my stuff!
Serious answer:
My friends have high ethical standards If I saved them at the expense of large groups of others I would never live it down.
Though I would have a very difficult time emotionally I would choose the greater good of saving many people.

It would pain me greatly, but I would have to sacrifice the person I am close to in order to save the world. If it were truly as perilous, then in saving my friend I would be dooming her anyway, as by taking that action I doomed us all.
Likewise, I would expect the same in reverse, if a person I esteem had to decide between me and the world, I hope he/she would choose the world.
