Neither. I'd complete the reference form honestly. A co-worker coming up to me and asking me to lie is, to say the least, a pretty big red flag for any future employer. It's not even the punctuality; it's the lying. There's a difference between wanting a reference to say positive things about you and wanting them to say something flat-out untrue -- and admitting that! What else would they lie about?
This might sound overblown, but these are things employers would want to know. And really, I'd be doing exactly what the form asked me to. I wouldn't be lying, and I'd be letting people know about the truth. I feel that's the right choice.
