First, I try to look at it for what it is...an expression of caring. Except in the case of "secret santas" and such at work and in other organizations, if someone doesn't care about you, they aren't going to give you a gift at all.
It's tough when it's from someone you really like or love, because you don't want to hurt them. The kindest way is to make up some excuse if you can, like you already have one. If it's something you can't do that with, like a piece of clothing, then tell them that it is lovely, but it's just not your style, so would they mind if you exchanged it.
If they don't have a receipt, or don't want to tell you where they got it (they may be regifting or it may be a clearance item....remember, there is a recession going on), then ask if they would be upset if you gifted it to someone who would like or could use it.

Honestly? I tell them to take it back. I've learned it doesn't REALLY hurt anyone's feelings. I'm the kind of person that asks for very specific things, and other than that I'd rather have nothing than have my family waste their money on garbage. My parents have never been rich, but they always spend an EXACTLY equal amount of money on my brother and me. He always asks for expensive things (games, a tv). The only thing I ever asked for that was expensive was a tv (that I never got), so I end up getting things like $50 dollar puzzles so my mom can spend her limit. What the heck do i need a 50 dollar puzzle for?! I don't need it, I don't want it, so why should my parents spend $50 on something I'll never touch. They might as well just keep the money.
If it's clear a lot of thought was put it, then I'll just shut up and keep it, and find some sort of use, but that rarely happens. But I don't need a 100 dollar ugly sweater, so bring it the heck back.
Maybe I'm just a bit of a downer at Christmas because i start thinking about money and I don't really need anything, so I see a lot of things as money wasted. Don't waste your money on me, please!
