this is tough - because either this person needs the fiver pretty badly or they are a clepto. Either way, they should be approached.
I believe that if I saw this happen, I would try to approach this person as a friend, especially if they have been in attendance at my church as a regular. Ask them if they need help, but in a tactful way.
If the person is not a regular, I would probably be a bit more direct and stern.
If this is ignored or not acknowledged, it could continue and that is not a good thing.

I would stop them right then, and ask them to come outside with me, where I would explain to them that the money in the collection plate belongs to God, and I could not let them steal from God and from the church. Then I would give him $5.00 out of my own pocket, as I assume they must need it pretty badly. I know, I do things differently form some folks.

I'd probably go find one of the people taking collections after the service -- at least at the church I used to go to, collections was a rather solemn time, people praying and whatnot, and I wouldn't want to make a scene right then. After the service is done and people have dispersed, I'd let them know what I saw and let them handle it.
I'm not a church official, after all. What they're doing is wrong, but it isn't my place to intervene directly. Besides, I don't know what's going on with that person. It looks bad, but they might just be really bad off financially, or panicked, or maybe it's all a misunderstanding. The people at church would know this much better than I would.
