I would go back to the store as soon as I realized that I had not paid for the item. I would not seek out the manager, I would go up to the cashier and pay for the item. It was an honest mistake and I would correct it as soon as possible.
A side note: I would "pay" much more should I decide to keep the item. Living with the guilt, rationalizing and justifying my behavior, no thanks, the freebie would cost too much.

Yes I go back and pay for it. Do I tell the manager? Not unless he is the one I see first. I simply would return the merchandise and pay for it and apologize while telling them that it was truly an accident.

I'd go back the second that i realised the mistake... I don't think it requires the manager, just the first person on the checkout that I can see. Then its just a case of "I'm sorry, I forgot to pay for this... " and thats it.

I would definitely go back and pay for it, because otherwise i'd have to live with the guilt. Even worse I'd get paranoid about someone seeing me do it and confronting later. All this would be really unpleasant, because although you stole something unintentionally in the first place, but by not returning it or paying for it you steal it "for real".

I would go get in line again and then pay for it. I may or may not tell the cashier depending on how friendly they looked, but there's no reason to talk to a manager. I guess some people may be worried about getting penalized for being honest, but I really don't think there's anything they could do. Besides, as long as you're cheerful and joking about it, it's going to be really hard for them to make trouble.

Ugh... it's happened to me a couple of times.
One time it was out of one of those big super-stores, so evidently their scanners were not working at the door.
I froze, pondered, considered how they were probably being managed by a bunch of corporate MBA brats, and how none of them would appreciate it if I went back in. In fact, they'd probably think I was nuts and would wonder why I didn't just keep going, because that's what *they'd* do... so I did.
The other time it was out of a small mom and pop operation, and I'd even made it to the car. In that case I turned around and went back and paid for it. They're small business, where little things like that can noticably affect their profits, and it's not like they're rich or anything... and yes, they *did* appreciate it. I got a free coffee for the trouble.
In terms of asking for the manager... I'm presuming you're asking, if I'd gone back to the small shop and asked to see the owner to tell them that one of their staff let me walk out without paying, and the answer is no.
I went strait to the person at the cash register, showed them what I had to pay for, watched them blush and look each way furtivly, and I accepted the free coffee after paying for the stuff.
So the answer is... depends on the situation.
